Section 1-5 of PIAA CIPPE must be completed on PlanetHS and signed by both parent and athlete to be cleared for participation.

Section 6 PIAA CIPPE must be signed by a MD, DO, CRNP, or SNP BETWEEN 5/1/25 and 4/30/26 to be cleared for participation. Athlete Name and Date must be clearly visible on Section 6.

Section 5 & 6 MUST be uploaded to BigTeams prior to preseason.
